The Transitional Living Room
Patients have access to an on-site transitional apartment where they have an option to spend the night. While staying in the TLR, patients will have the opportunity to practice independent skills needed to return home safely while being coached by staff in a supportive environment. The TLR is also used as a place for families to stay with patients so that they can practice caring for their loved one in a supportive environment before they return home.
Community Outings
We help build skills and confidence with practice trips to the store, bank or restaurant.
Family Involvement
The Clinical Liaison meets with the patient and family before arriving, answering questions about the program and facility. Upon admission to the program, our team of rehab experts involve patients and family in care plan meetings, training and discharge planning to ensure goals are met and the patient and family are comfortable with returning home.
Devoted Teams
A team of experts will help each individual reach their personalized goals.
Weekly Meetings
The rehab team meets daily, discussing each patient's unique needs, goals and progress. Then, each week, the rehabilitation team will set goals with patient input so that each patient can return to their daily activities as quickly as possible.
Stroke Education Program
A stroke education program is provided for patients and family. During the one-hour sessions, we provide information on how to prevent a secondary stroke.
Home Safety Reviews
We review the patient's home setting for safety and accessibility, and may make a home visit as needed to ease their move home.
Transportation
We offer free transportation to and from our facility.
